Registration
As summer approaches, we invite you to come and meet us at our Family Open House, on Saturday June 19 in Ste-Agathe-des-Monts, where all families with a diabetic child are welcomed. To this effect, you can download the information and schedule below.
The 2010 registration forms have been mailed or sent by email in January to all families-members of The Diabetic Children’s Foundation, offering three two-week sessions of activities for young diabetics 8 to 16 years old or a family weekend for children under age 8 and their family.
If you have not received your registration form, please advise us or download the form below.
Registrations are processed on a first-come, first-served basis.
If you need financial assistance, do not hesitate to request or download the confidential financial-aid application form below. The information provided will in no way compromise your child’s acceptance as a camper and will be kept strictly confidential. Please return all the duly completed and signed forms as soon as possible, and at the latest by April 30, 2010 to the following address :
